Registration

New Patient Registration

All persons making application to join our Practice list must do so by requesting an application form from our reception staff.

Please check with reception staff that you live within the practice boundary or click here to view a map.

Completed forms should be returned to reception to facilitate registration.

Should registration not be acceptable the practice will provide a reason, in writing, within 10 working days.

Patients may require a new patient check; the reception staff will arrange an appointment for this.

Patients may make an appointment with their doctor prior to a new patient check appointment if they wish.

 Changes to Personal Details

It is important that we have up to date details in your medical file.

Please inform the surgery of any changes i.e. you change address or change name.
